Request a Policy 4105 Waiver

In the event that a publisher refuses to acknowledge the prior non-exclusive license between you and Utah State University, Policy 4105 may be waived at your sole discretion on an article by article basis. To waive Policy 4105, simply complete the form below and a waiver that you may keep for your records will be automatically emailed to you. Be aware, however, that by waiving Policy 4105 and granting exclusive rights to your work to your publisher, you will greatly reduce the rights you retain to your own works.
